What goes on a fax cover sheet
A fax cover sheet is the first page of a fax. Its job is routing: a fax arrives at a machine or shared inbox, not at a person, so the cover tells whoever picks it up where it should go and whether anything is missing. A complete cover sheet includes:
- Recipient — name, company or department, and their fax and phone numbers.
- Sender — your name, company, phone, fax, and email, so the recipient can reach you if pages are missing or unreadable.
- Date and page count — the total number of pages including the cover, so a short transmission is obvious.
- Subject (RE) — a case, claim, account, or patient reference number if you have one. This is what gets your fax matched to the right file.
- Action boxes — Urgent, For review, Please comment, Please reply, Please recycle — a quick signal of what you expect.
- A short message and, where appropriate, a confidentiality notice.
Confidentiality notice examples
The generator pre-fills a standard notice you can edit. Shorter or more specific versions work too:
Short: This fax is intended only for the named recipient and may contain confidential information. If you received it in error, please notify the sender and destroy all copies.
Legal: This transmission may contain information protected by the attorney–client privilege or the attorney work-product doctrine. If you are not the intended recipient, any review, use, or distribution is prohibited. Please call the sender immediately and return or destroy the original.
Financial: This fax contains personal financial information intended solely for the addressee. Do not copy or forward it. If received in error, contact the sender at the number above and destroy this document.
A note on HIPAA and medical faxes
Healthcare providers fax protected health information every day, and a cover sheet is part of doing it carefully. Commonly used wording looks like this:
This facsimile may contain protected health information that is privileged and confidential. It is intended only for the use of the recipient named above. If you are not the intended recipient, you are notified that any review, disclosure, copying, or distribution is prohibited. If you received this fax in error, please notify the sender immediately by telephone and destroy the documents.
Keep the patient's details off the cover sheet itself — put them on the following pages — and double-check the destination number before sending. This is general guidance, not legal advice; your organisation's privacy officer sets the wording and procedures you should actually use.

Frequently asked questions
Do I need a fax cover sheet?
It is not legally required, but it is strongly recommended. Many offices share one fax machine or a single inbound fax number, and the cover sheet is how your pages reach the right person instead of sitting in a tray. Hospitals, courts, and government agencies often expect one.
Is this fax cover sheet generator really free?
Yes. Filling in the form and downloading the PDF is free, with no account and no watermark beyond a small footer line. You only pay if you choose to send the fax through Faxio, which costs $2.99 per fax with no subscription.
Is my information uploaded anywhere?
No. The PDF is generated entirely in your browser. Nothing you type is sent to our servers unless you click Send with Faxio, in which case the cover sheet is attached to the fax you are about to send.
Does the cover sheet count toward the page total?
By convention, yes. Write the total number of pages including the cover sheet, so the recipient can tell immediately whether every page arrived.
What size is the PDF?
US Letter (8.5 × 11 inches), in black and white. It prints cleanly and transmits well at standard fax resolution, where colour and fine grey shading tend to turn into noise.
Should I include a confidentiality notice?
If the fax contains personal, financial, legal, or medical information, a confidentiality notice is a sensible default. It does not make an intercepted fax legally protected on its own, but it tells an accidental recipient what to do and documents that you treated the information as confidential.
