There is no national DMV, and there is no general DMV fax number. Each state runs its own motor vehicle agency, and inside each agency fax is usually accepted only by specific units for specific documents. Fax your licence renewal to a medical review line and nothing happens. Fax a medical report to the right unit and it can stop a suspension.

This guide explains how to fax documents to the DMV: what is typically accepted by fax, what is not, and verified fax numbers from four state agencies as examples of how it works.

  • Fax is unit-specific. Medical review and insurance compliance are the common cases.
  • Most transactions are not faxable, including renewals, titles, and registrations.
  • Verified examples from Florida, New York, Virginia, and Texas

What DMVs usually accept by fax

Fax tends to survive in the parts of a motor vehicle agency that deal with documents from third parties, such as doctors and insurers, rather than with walk-in customers:

  • Medical review forms. When a state reviews whether a driver can drive safely, it often asks for a medical report completed by a physician. Many medical review units accept these by fax.
  • Vision reports. Eye test reports from an eye doctor, for drivers referred for vision review.
  • Proof of insurance or compliance in response to a suspension notice, where the state lists a fax option.
  • Responses to a specific letter that gives a fax number for that purpose.

The common thread: you are responding to a request, and the request tells you where to send the answer.

What you generally cannot fax

Most everyday DMV transactions need payment, original documents, a photo, or identity checks, so they are done online, by mail, or in person:

  • Driver licence and ID card renewals
  • Vehicle title transfers and new registrations
  • Applications needing original identity documents, such as a REAL ID
  • Anything that needs a fee paid at the time of filing

If the form or letter you are holding does not mention fax, assume it cannot be faxed and check the agency's website.

Verified DMV fax numbers (examples)

These examples come from each state agency's official website or forms. They show the pattern: fax numbers belong to a specific unit, for a specific kind of document.

Verified October 2026:

State and unit Fax number What to fax
Florida FLHSMV, Bureau of Motorist Compliance, Medical Review Section 850-617-3944 Medical reports for medical review
Florida FLHSMV, Bureau of Motorist Compliance, Vision Section 850-617-3936 Vision reports
New York DMV, Medical Review Unit 518-402-2991 Medical review documents, such as the MV-80L eye test report
Virginia DMV, Medical Review Services 804-367-1604 Completed MED 2 and MED 4 medical forms
Texas DPS, Driver License (Safety Responsibility) 512-424-2848 A letter from your insurance company, on company letterhead, showing you had insurance on the date in question

Details worth knowing:

  • Virginia asks for MED 2 and MED 4 forms at least 5 to 7 days before the customer's suspension date, and also accepts them through an online portal or by mail.
  • New York's Medical Review Unit lists its mailing address as 6 Empire State Plaza, Room 337, Albany, NY 12228, and its page does not list fax instructions form by form. If you are not sure a form can be faxed, call the unit at 518-474-0774 (option 2).
  • Texas says this insurer letter applies if you had insurance at the time of the citation, and to allow 21 days for processing. Texas also states it does not accept a copy of your policy or insurance card in place of an SR-22.

These are examples, not a complete list. Other units in these states, and every other state, use their own numbers.

How to find your state's DMV fax number

  1. Read the letter or form. A medical review request, suspension notice, or compliance letter normally names the fax number and the unit.
  2. Go to the agency's official site. Search the state's DMV or motor vehicle agency site (it ends in .gov) for the unit by name, such as "medical review" or "insurance verification".
  3. Check the form itself. State medical forms often print the fax number at the top or bottom.
  4. Call the unit if the form does not say whether fax is accepted.

Avoid numbers from third-party "DMV help" sites. Several look official and are not, and a fax to a wrong number sends your medical or personal details to a stranger.

How to fax a document to the DMV

  1. Confirm fax is accepted for this document and unit.
  2. Complete the form fully. Medical forms usually need the physician's signature, licence details, and date. An unsigned form is sent back.
  3. Add your identifiers to every page: full name, date of birth, driver licence or customer number, and any case number from your letter.
  4. Scan clearly in black and white. Handwritten physician notes and checkboxes need to survive fax resolution.
  5. Send to the unit's fax number, well before any suspension date.
  6. Keep the confirmation with a copy of what you sent.

You do not need a fax machine. You can send a fax online from a browser for a one-time payment, with no subscription and no account, and get the delivery confirmation by email. That is often easier than asking a busy doctor's office to fax a form for you: get the signed form back, then send it yourself.

Medical information and privacy

DMV medical forms contain health information. Under HHS guidance, health care providers may fax health information with reasonable safeguards, and confirming the destination number is one of them. When you send the form yourself, the same habit protects you: check the number against the official source before you send.

Faxio stores uploaded documents encrypted and uses them only to render and transmit your fax, as described in our privacy policy.

Faxing the DMV works when three things line up: a document the unit accepts by fax, the unit's own fax number from the state's official site or your letter, and a confirmation you keep. If any of those is missing, check the state's website before sending.

Frequently asked questions

Can I fax documents to the DMV?

Sometimes. Most DMVs do not take general paperwork by fax, but many accept specific documents at specific units, such as medical report forms for medical review or proof of insurance in response to a suspension. The letter or form you received will say whether fax is an option.

Can I renew my driver's license by fax?

Generally no. Licence renewals, title transfers, and registrations are handled online, by mail, or in person, often because they require payment, original documents, or a photo. Fax is mainly used for medical review and compliance documents.

Who can fax a medical form to the DMV?

Usually either you or your doctor's office, depending on the state and the form. The completed form needs the physician's signature and details, and it should go to the medical review unit's fax number printed on the form or the state's medical review page.

Can I fax my SR-22 to the DMV?

Usually not yourself. Texas DPS describes an SR-22 as a certificate filed by your car insurance carrier, so the insurer handles it. Separately, Texas DPS accepts a letter from your insurer by fax at 512-424-2848 to show you had insurance on the date in question.

How long does the DMV take to process a faxed document?

It varies by state and unit. Texas DPS, for example, says to allow 21 days for processing after it receives an insurance letter. Send early if a suspension date is approaching.
